The team at Transformations Treatment Center is committed to your continued recovery and growth. We believe in equipping graduates with the necessary tools to achieve long-term sobriety.

Here at Transformations Treatment Center we are proud of our Guaranteed Success Program!

Transformations Treatment Center has created a well-developed discharge program for our graduates.

Aftercare Plan

• Staying Connected: The addictions counselors and Alumni Department at Transformations Treatment Center remain in touch with program graduates to help them stay connected with resources once they leave treatment. We find that graduates who have familiar people to turn to in times of distress will successfully work through a crisis.

• Group Therapy: Any graduate of Transformations Treatment Center is welcome to attend as many as nine group sessions a week for free during his or her first year of sobriety. Graduates must follow their aftercare treatment plan to be eligible for this program.

• Alcoholics/Narcotics Anonymous: Whether graduates remain local, or travel back to their hometown, our Program Coordinator will help clients develop personalized continuing care plans to include weekly AA/NA meetings and support groups.

• Celebrate Recovery: We also help the graduates of our Christian Rehabilitation Program coordinate with churches and other ministries that offer Celebrate Recovery meetings.

• Sober Living: After treatment, each graduate is encouraged to continue his or her recovery in a Halfway House that Transformations Treatment Center has interviewed, toured, and approved.

We are so focused on helping graduates remain in recovery that any individual who has graduated from our 90-day program and followed our aftercare program is eligible for an additional 28 eight days of treatment should they relapse within the first year. There is no cost to the individual should they come back for treatment except for detoxification and transportation if necessary.

Disclaimer

Individuals who have successfully completed a treatment program at Transformations Treatment Center and have chosen to live at a local sober living home must follow the Aftercare Plan set by their primary therapist in order to be eligible for the Guaranteed Success Program. Client must complete aftercare program designed by Transformations Treament Center for eligibility. Client must also be medically cleared by a detox center before coming back into Transformations relapse prevention program. Any client that recieves a full or partial scholarship will not qualify.
